Dear Colleague, I am writing to refer Mrs. Jones, who has presented with abdominal discomfort for the past two to three months. The discomfort is exacerbated by the consumption of fatty foods. Additionally, she has observed that her stools are slightly looser and paler than usual. Importantly, she denies any weight loss or altered bowel habits. Mrs. Jones is generally fit and well, but she has a history of depression for which she takes the following medication: • Citalopram 10 milligrams daily She has an allergy to: ALLERGY: PENICILLIN Mrs. Jones does not smoke and consumes alcohol socially. There is no family history of significance noted. On physical examination, her abdomen was generally unremarkable, although it was slightly distended and minimally tympanic, with tenderness in the right upper quadrant. Her temperature was recorded at 36.6 degrees Celsius, and her blood pressure was 142 over 77. Oxygen saturation was noted to be 98. I would be grateful for your assistance with this lady. She does not smoke and drinks alcohol socially. No family history of significance. Physical examination was generally unremarkable with a slightly distended abdomen that is minimally tympanic with tenderness in the right upper quadrant. Her temperature was 36.6 degree Celsius.
